This agreement, entered into this ________________________ day of ______________________ year of __________________________ by and between consultant, whose name and address are listed above and ____________________________________________________who resides at______________________________________________ Client’s Phone # ( _______) - _________ - __________________.Herein after referred to as "Client", agrees as follows:• Consultant will analyze and investigate his/her personal credit report.• Consultant will prepare all the necessary correspondence for clarification of certain derogatory information which may be contained in a client’s credit report.• TR & Associates Credit Service program will not exceed 60 days, due to certain time constraints imposed by State and Federal laws on creditors and credit reporting agencies.• Client agrees NOT TO APPLY FOR CREDIT until the completion of this agreement.• Client is aware that all derogatory items after a specific period of time must automatically be removed. Bankruptcies: ten years from date. All others seven years from the date of last activity.• Client is aware that there are, available to him/her, nonprofit credit counseling services, and to check local listings.• Client further agrees to provide the consultant with a copy of all correspondence received by the client as a result of the consultant’s efforts and/or processing.• Client is responsible for all debts owed to creditor.• If client feels he/she has been damaged by a violation of Sec. 4712.01-4712.14 OH.R.C. Client has the right to proceed against the bond #610220696-4 held at: Schiff/Kreidler/Shell • 305 Vine Street • Cincinnati, Ohio 45202.• "IF YOU, THE BUYER, HAVE BEEN DENIED CREDIT WITHIN THE LAST SIXTY DAYS, YOU MAY OBTAIN A FREE COPY OF THE CONSUMER CREDIT REPORT FROM THE CONSUMER CREDIT REPORTING AGENCY. YOU ALSO HAVE THE RIGHT TO DISPUTE INACCURATE INFORMATION IN A REPORT. YOU MAY CANCEL THIS CONTRACT AT ANY TIME BEFORE MIDNIGHT OF THE THIRD BUSINESS DAY AFTER THE DATE YOU SIGNED IT. SEE THE ATTACHED NOTICE OF CANCELLATION FORM FOR AN EXPLANATION OF THIS RIGHT."• Client understand’s�• With respect to the previous calendar year, TR & Associates Credit Services has fully completed 100% of services we agreed to perform and completed for our customers.• Consultant, at his/her discretion, may keep for his/her records all correspondence and material used and produced by him/her.• Client agrees that this constitutes their entire agreement and understands the consultant merely sends out letters and faxes.• The statutory agent in this state authorized to receive services of process, Todd F. Ward at 8080 Beckett Center • Suite 204 • West Chester, Ohio 45069.• Consultant will send letters to the credit bureaus by utilizing methods in the FAIR CREDIT REPORTING ACT OF 1970, SECTION 168Li. However, the consultant does not promise any particular standard of improvement can be attained. This will remain in effect only if you abide by the following: 1. Have no checks returned from your bank for any reason. 2. Mail all disputation letters promptly. 3. Do not apply for credit while this agreement is in effect.There is a $20.00 fee for all Returned Checks! All returned checks must be replaced with cash or cashier’s check within 24 hours.FEE: TOTAL FEE IS $446.00 FOR AN INDIVIDUAL PERSON. $649.00 FOR A COUPLE. THIS FEE IS PAYABLE WHEN THE FOLLOWING FUNCTIONS ARE TOTALLY COMPLETED: 1) INITIAL CONSULTATION. 2) INTERPRETATION OF CREDIT REPORTS. 3) PREPARATION OF LETTERS TO BE SENT TO CREDIT BUREAUS.1.
